Shadow warrior is the top perk for sneak skill tree. It's basically makes you invisible for a very short duration maybe seconds whenever you initial crouch into sneaking.

This enables you pull of multiple sneak attacks if you time it right since the enemy loses sight of you temporarily. If you slow the effect down with slow time shout or quick reflexes perk you'll have a moment longer to set it up right.

You're follower is independent of you but they can make things more difficult. Wear gear that boosts sneaking or invest more perks into sneaking more effectively. Boots with muffle enchant are awesome and don't forget to grab the light armor perk that makes light armor weightless.
